Name
This is the form’s internal identification. It helps you recognize it in the listing and lets the customer know what it is about when they open it. Keep it short and specific: “Excel Training Registration” or “Billing details”.Fields
Every Form has an ordered list of fields. For each one you define:- The type of data to be captured.
- The label the customer will see.
- Whether it is required or optional.
- Placeholder (guide text inside the field, optional).
- Options, when the type requires them (select and multiselect).
- The order within the form.

select and multiselect types require you to define the list options.
Order and required status
The order determines how the form is presented to the customer. The required status blocks submission if the field is empty. Try to keep required fields to a minimum so you don’t scare off the customer, and leave everything that is not strictly necessary as optional.You can freely edit a Form as long as it has no responses. After receiving the first response, you will not be able to add or remove fields or change their types. You will be able to adjust their text, options, required status, and order.
Multi-language forms
The same Form can be available in Spanish and English under the same link. When you enable another language, you can write the translations manually or let woku generate them with AI. Afterward you can review or modify them from the settings. Responses from all languages are consolidated into the same Form. See Multi-language surveys to learn how they work.Settings
The form’s three settings define how it behaves:Anonymous responses
If enabled, the customer can submit the form without providing any contact information. If disabled, the form requires the customer to identify themselves according to the chosen identifier.Customer identifier
Defines how the customer is identified when they do not respond anonymously. It can be:- Email: they are asked for their email address and are sent a verification email.
- Phone: they are asked for their phone number.
One response per customer
If enabled, the same customer cannot submit two responses to the form. Useful for registrations, single sign-ups, or cases where you need to avoid duplicates.Response verification by email
When the customer responds with their email address, the form first creates a provisional response that is stored for up to 7 days while the customer verifies their email. When they verify:- The response becomes a final response.
- It is associated with the customer and available in the reports.
- If verification never happens, the provisional response is automatically deleted after 7 days.
Form status
A Form can be:- Active: it receives responses.
- Inactive: it does not receive new responses. Previous responses remain available.
- Closed: same as inactive, but explicitly communicated to the customer.
Sharing
Same as a woku, an NPS, or a Flow. Three options:- QR code.
- Link.
- Bulk email send.
Responses
Responses are available in the Form view. For each response you will see the values of each field, the submission date, and, where applicable, the customer’s contact information. You can export everything to CSV to process it in a spreadsheet or load it into your system.AI analysis
When the Form accumulates at least 5 responses, open the Summary tab to generate an AI analysis of all available responses. The report includes:- A general summary of the volume and the analyzed period.
- Patterns, segmentations, and anomalies across the fields.
- Actionable conclusions.
- Statistics per field, such as averages for numeric values and frequencies for options or text.
